Download iCal FileThis presentation will describe the origins and conceptual framework of Transformational Collaborative Outcomes Management (TCOM) and comprehensive approach to creating and evolving systems that engage in person-centered care. This approach and its associated tools (Child and Adolescent Needs and Strengths, Adult Needs and Strengths Assessment, and Family Advocacy and Support Tool) are now used in at least one helping sector in 38 states and on every continent. The measurement theory, Communimetrics will also be described as it creates a measurement approach that supports person-centered care. In 2019, approximately 80% of all children and youth served in the public sector in the US will be touched by the CANS providing us with a unique opportunity for learning together how to best serve families in need.
